Omal gives a dissertation on the
pervasive sloppiness inherent in the culture of
NASA at the time. He brings up the Challenger
disaster as his analogy where lessons were not
learned. The shuttle Columbia’s flight into space
that is mentioned is the example he uses where
serious mistakes were made but ignored for
politics at NASA. Nearly four years later on its
next flight, the shuttle disintegrated on reentry
after an exploratory spacewalk to review the
damage was denied by the management of
NASA.
